In this fully remote role, you’ll be a vital lifeline—connecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.

What You’ll Do

Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.
Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.
Early Intervention: Monitor symptoms, identify risks, and escalate care before complications arise.
Coordinate Care: Work seamlessly with primary care providers, specialists, and community resources.
Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
Document with Precision: Maintain accurate and timely patient records in our EHR system.
Stay Compliant: Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.

What You Bring

Valid RN license in the Philippines
Minimum of 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus)
Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure
Excellent communication, critical thinking, and organizational skills
Comfort using EHR systems and virtual care tools
Ability to work independently while thriving in a collaborative team environment

Bonus Points If You Have

Familiarity with Medicare Chronic Care Management documentation and compliance
Experience with motivational interviewing or other patient engagement techniques
